1. 1Bihar Labour Department ki website (state.bihar.gov.in/labour) se ya Labour Inspector office se form lein.
2. 2Yeh unke liye hai jo khetihar mazdoor, thela chalane wale, ya asangathit kshetra (unorganized sector) me aate hain.
3. 3Form me aavedak ko apna basic details aur nominee ki details bharni hoti hai.
4. 4Local mukhiya ya ward councillor se sign karwa kar pramanit karein ki aavedak asangathit mazdoor hai.
5. 5Aadhar aur bank account attach karke form Block Labour Enforcement Officer (LEO) ko jama karein.
6. 6Is scheme me mazdoor ki samanya mrityu (natural death) hone par nominee ko ₹30,000 aur accident me maut hone par ₹1,00,000 ki sahayata di jati hai.
